PRIVACY POLICY
STATEMENT OF INTENT
This policy sets out how Solutions with Sarah Hypnotherapy will use and protect the information that you provide when you use my services and website.
It is my intention to ensure that any data you provide is managed respectfully, kept secure and only used for the purposes for which it has been provided.
This policy will be updated from time to time in line with prevailing legislation.
HOW I GATHER YOUR INFORMATION
When you contact me via my website, phone or email, I will collect your name, email address, telephone number and any information you choose to supply regarding the purpose of your enquiry
During your Initial consultation, I will use a Data Gathering document and will ask for, or confirm if already received, your name, address, telephone and email contact details so that I may contact you during the time we are working together, what you would like to achieve as part of your Therapy sessions, your GP contact details and health information: there are some conditions which are contraindicated for hypnotherapy
ONLINE SESSIONS
Regarding sessions conducted online, I use Zoom. This software has a facility to record sessions which I will NOT use. The link I will send you to our online session will also come with a passcode, meaning no one else can join this private session.
USES MADE OF THE INFORMATION
Over the course of our sessions I will make session notes from each session in order that I can track your progress and plan future sessions.
WHERE I STORE YOUR PERSONAL DATA
Personal data that I collect from you via any form of contact by email or phone will be kept securely.
Your contact details, and brief session notes are stored in a fireproof locked cabinet in my therapy room.
The brief session notes are anonymized, using your initials only.
I take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy.
HOW LONG I KEEP YOUR PERSONAL DATA
I will retain your Initial Consultation Data Gathering questionnaire and session notes for as long as we are working together and then for a further five years after we end in case you decide to return to therapy with me. This is also a requirement of my indemnity insurance.
YOUR RIGHTS
Under the General Data Protection Regulations which are effective from 25th May 2018 you are entitled to view, amend, or delete the personal information that I hold. All requests have a month to be carried out.
SHARING DATA
Solutions with Sarah Hypnotherapy is the data processor for the purposes of administration (including appointment making) and accounts. As the data processor, I am trained for GDPR purposes and fully compliant with the requirements.
Solutions with Sarah Hypnotherapy premises may operate CCTV surveillance for the purposes of security. Any data obtained via CCTV is protected in accordance with the GDPR
DISCLOSURE OF YOUR PERSONAL INFORMATION
In the event of my incapacity or death your personal contact information will be disclosed to my Professional Supervisor who will notify you and will destroy all contact information and notes stored.
Disclosure could occur if I am under a duty to disclose or share your personal data in order to comply with any legal obligation. For example, if I am requested to attend court, or as a legal requirement such as safeguarding children or vulnerable adults, terrorism or money laundering.
